Linking Interest with needed Kare (LINK) - Adult day care center that offers activities that help to retain skills and increase independence, an LPN is on staff at all times. Service Contact Person: Amy Henson

Need physical (including TB tine test) and plan of care
Eligibility
Age 18 years and older with any illness or disability that leaves the individual incapable of being left alone

Private Insurance Medicaid Private pay ($10 per hour)

Referral required from Missouri Dept. of Health and Senior Services if Medicaid is the payee
