- Disconnect Notice - Photo ID for each household member age 18 years and older - Proof of Income (paycheck stubs, Documentation on company letterhead stating the number of hours worked per week, Most recent SSI, Social Security Disability Benefit Statement, Documentation that client is currently registered at Missouri Career Center - Copy of LIHEAP Grant Letter (If applying November 1 through March 31 only)
Eligibility
- Client may receive one energy assistance grant per year per household - All required documentation must be in order and up to date, and client must meet the Loving Hearts Outreach financial guidelines - Utility Account must be in the name of Client applying for assistance
