- Ryan White Case Management: Provides linkage to primary care, medications, housing, food, and other supportive services for individuals with HIV. - BEACON Project: Provides an intensive team based approach to engage clients into the system of care. The team consists of a Case Manager, an Advocate and a community nurse who work to reduce the barriers clients face when engaging and retaining medical care. Clients are enrolled into the Ryan White system of care and followed by the team having access to additional resources to assist in reducing barriers.
