- Valid Picture ID (Driver's License or Passport) - Social Security Card for everyone listed on the tax return, letter from Social Security office of Individual Taxpayer Identification Number (ITIN) - All W-2's, 1099-R, SSA-1099 and other income - For W-2G (Gambling Winnings) provide statement or proof of gambling losses - Bank interest 1099-INT/Dividends 1099-DIV - 1099G - Unemployment Income - Copy of 2013 tax return (if didn't file at this location) - Paid documents or statements for Schedule A (Itemized Deductions), including all charitable contributions from church or organization - Paid document for residential energy efficient credit or qualified deduction or expenses - Receipt for daycare expense (name, address, SSN or EIN of childcare provider) - Form 1098T for Education Expense or Credits, student ledger sheets school cashier's office with breakdown of fee and tuition assessments - Clients claiming Circuit Breaker - signed letter from property owner of rent paid by tenant to include property owner name, address, phone number, and taxpayer identifying number; paid rent receipts; paid real estate tax bill; other household income sources such as VA benefits, Social Security or SSI benefits received by other household members - Bank Routing and Account Number for direct deposit to checking or saving account or reloadable debit card or payment for tax balance due - If married filing jointly - both spouses must be present to sign tax returns
