Program designed to provide a standardized oral health screening for children in the community: To provide at the time of screening: oral health educational information and instruction, an electric toothbrush, and application of fluoride varnish. To provide a structure to organize community response to the rising need for oral health services for all populations.

Description

The goal of rapid re-housing is to serve individuals and families who are literally homeless (people residing in shelters). The assistance will transition eligible participants into permanent housing as quickly as possible with the intention of helping such persons achieve stability in their housing. This program will provide long term rental assistance to assist individuals get established into permanent housing. Participants will work with a case manager to establish goals that will increase their abilities to maintain permanent housing on their own.
