Permanent Supportive Housing Program: For young adults who have aged out of foster care. The goal is for clients to move towards self-sufficiency by focusing on housing, health, well-being, education, employment and more.

- Referral from Epworth - Photo ID - Social Security Card - Proof of Income/Employment - Banking information - Education History - Criminal background check

Eligibility

- Referred from Epworth - Ages 18-24 years at time of admission, formerly in foster care, aged out or no longer in care - Must make $387.50 a month in verifiable income without exceeding $32,800 in annual income - No unsettled landlord action - No drug conviction for the past 3 years
