Provide assistance with Medicare Part D and Missouri Rx application and enrollment process. Can also advocate for clients experiencing any problems with those programs. Service includes case/care management.

Required Documents
Must complete an agency registration form (NAPIS) and assessment forms. Social Security Card, Proof of Residence, Medicare Card might be required to apply for some benefits
Eligibility
Individuals aged 60 years of age and older or those asking on behalf of someone aged 60 years of age or older
Fees
Voluntary contribution toward cost of service is requested - Individuals that do not meet requirements must pay full cost
