Description

Case plans are made for individuals with severe and persistent mental health challenges to assess needs, coordinate needed services, ensure services attained and to follow up and monitor progress.

Hours

M-F 8am-4pm

Required Documents

Please bring with you, if applicable, a photo ID, insurance card, guardianship paperwork, and birth certificate (for minors).

Eligibility

Severe and persistent mental illness that impacts the person's ability to manage their social role or daily living skills

Fees

Medicaid, Medicare and most types of commercial insurance except for Cigna and United Health. Missouri Department of Mental Health funding for uninsured individuals who qualify.

Intake Procedure

Walk-in care is available Monday through Friday from 8am – 12pm. A master's-level mental health professional will meet with you and help you get the care you need. Each person who utilizes walk-in hours will be assessed and connected to appropriate services.

Languages

Arabic, Somali and Spanish. Phone- and video-based interpreter services are available for other languages, including American Sign Language.
