Blood Pressure Self-Monitoring is an evidence-based hypertension management program that utilizes blood pressure self-monitoring to empower participants to take ownership of their cardiovascular health. With the assistance of a Healthy Heart Ambassador participants learn to manage their blood pressure by developing the habit of routine self-monitoring and tracking readings. Participants choose the program format that fits them best; available formats are one-on-one or group.
