Required Documents
Must complete an agency registration form (NAPIS) and assessment forms. Social Security Card, Proof of Residence, Medicare Card might be required to apply for some benefits
Eligibility
Individuals aged 60 years of age and over, or those asking on behalf of someone aged 60 years of age and over
Fees
Voluntary contribution toward cost of service is requested - Non-eligibles must pay full cost
