Description

Required Documents
Must complete an agency registration form (NAPIS) and assessment forms. Social Security Card, Proof of Residence, Medicare Card might be required to apply for some benefits
Eligibility
Anyone who is 60 years of age and over - Or those asking on behalf of someone 60 years of age or over
Fees
Voluntary contribution toward cost of service is requested - Non-eligibles must pay full cost
