Description
Blood Pressure Self-Monitoring: With the assistance of a Health Heart Ambassador participants learn to manage their high blood pressure by developing habits of routine self-monitoring and tracking. Participants also attend two personalized consultations per month and attend monthly nutritional education seminars each month of the program.

- Must be at least age 18 years and older - Must be diagnosed with high blood pressure - Must not have experienced a recent cardiac event - Must not have atrial fibrillation or other arrhythmias - Must not be at risk for lymphedema
